Manistee County Community Foundation to Support NorthStar Palliative Care

Grant to help expand community-based palliative care services to the Manistee community to better support patients and families living with advanced or life-limiting illnesses

ANN ARBOR, Mich. (June 20, 2024) – Hospice of Michigan recently received a $10,000 grant from the Manistee County Community Foundation to help expand NorthStar Palliative Care (NSPC) to the Manistee community. The grant will help serve local patients and families living with advanced or life-limiting illnesses.

NSPC is a community-based palliative care program that provides symptom management and advanced care planning services for patients who have been discharged from a post-acute setting to home. The program offers an additional layer of support in collaboration with the patient’s primary care physician and other treating specialists, until the patient is either eligible, no longer seeking aggressive treatment or ready for hospice care. NSPC supports caregivers and collaborates with physicians, health care organizations, and community organizations to ensure access to high quality, palliative care services in the home.

“Manistee County traditionally has not had access to home-based palliative care services for patients living with chronic illnesses,” said Roxanne Kiesling, philanthropy manager at Hospice of Michigan. “Thanks to this gift from the Manistee County Community Foundation, Manistee residents will now have increased access to this essential care – improving their quality of life near end of life.”

This program is made possible in part by a grant from the Manistee County Community Foundation – Healthy Youth Healthy Seniors Fund and the Manistee County Community Foundation – Manistee County Quality of Life Fund.
