The expert Hospice of Michigan Care Team includes:
- Your physician: The patient’s physician remains involved and plays a crucial role in developing a personalized plan of care.
- Physicians Board-Certified in hospice and palliative medicine collaborate with the patient’s physician to manage care. This advanced training in pain management and symptom control allows Hospice of Michigan physicians to prioritize reducing suffering and maximizing quality of life.
- Advance Care Practice Nurses: Skilled and credentialed nurse practitioners and nurse navigators work closely with physicians to support hospice and palliative care patients.
- Registered Nurses: Specially-trained hospice nurses are on call 24-7. They visit as often as needed to monitor a patient’s condition and provide comfort care. They provide day-to-day care, assessing the physical condition and comfort of the patient. They help manage symptoms and pain.
- Social Workers assess patients’ and families’ emotional, social, spiritual, and financial needs and arrange essential resources for the family. They provide counseling, educational, and emotional support to patients and their families during this challenging time. Social workers also assist with specific end-of-life issues, such as having to say goodbye, life review and validating the importance of each patient’s life.
- Integral to the care team, Spiritual Care Advisors hold sacred space for patients so that they can connect with their beliefs, find peace, meaning, acceptance, or reconciliation, no matter what faith they practice.
- Hospice Aides offer much-needed relief for families and caregivers by helping with the patient’s personal care and everyday activities.
- Grief and Bereavement Support Service Managers help family members of all ages throughout care to better understand and manage the grieving process. Grief support is available up to 13 months following a patient’s death.
- Trained Volunteers give their time generously to provide quality of life support to patients and their loved ones. Volunteers assist in a multitude of ways, including companionship, respite care, running errands, and grief support to patients and families as needed.
- Hospice Care includes prescription and over-the-counter medications, medical equipment and supplies associated with the terminal illness, delivered right to the patient’s home.
