Grant from the Randall and Linda Wagner Family Fund of the Oceana Community Foundation to benefit patients and families living with advanced or life-limiting illnesses
ANN ARBOR, Mich. (June 18, 2024) – Hospice of Michigan recently received a $5,000 grant from the Oceana Community Foundation. These funds, awarded from the Randall and Linda Wagner Family Fund of the Oceana Community Foundation, will support the not-for-profit’s NorthStar Palliative Care (NSPC) program.
NSPC is a community-based palliative care program that provides symptom management and advanced care planning services for patients with advanced or life-limiting illnesses who have been discharged from a post-acute setting to home. The program offers an additional layer of support in collaboration with the patient’s primary care physician and other treating specialists, until the patient is either eligible, no longer seeking aggressive treatment or ready for hospice care. NSPC supports caregivers and collaborates with physicians, health care organizations, and community organizations to ensure access to high quality, palliative care services in the home.
